I don't believe John Silber embezzled massively from BU or engaged in pedophilia. On the other hand, neither Berendzen nor Ladner actively drove famous faculty away as Silber did (or indeed Lawrence Summers at Harvard). But, finally, neither of them are in the pretension ballpark because they both presided over more prestigious institutions in the first place. Their sins were more arrogance than buffoonery and venality.

In my case, I'm pretty sure it's due to inexcusable sloppiness. Your larger issue about rigid orthography may explain why we should not get worked up by such slips. But it doesn't excuse the laxness of pedants when they make them.

I've read Lacan, but I've never read what his patients say. Where does one find that. I should say that, while I sort of understand Lacan's basic argument, I frequently can't actually decipher what he writes. I always wish, like the letter in Poe, his meaning had been hidden in plain sight, instead of just hidden.
