Anne Gros makes a Combe d'Orveau that is a Villages while others, e.g. Faiveley, make Combe d'Orveaus that are 1er Crus. Wot gives? If Anne Gros is using Combe d'Orveau as a lieu dit, it seems strange that the "authorities" would authorize a lieu dit with the same name as a 1er cru vineyard...

Not strange at all - well, maybe strange but not unusual. A portion of Combe d'Orveau is classified as premier cru and a portion is not. Another example is Veroilles also in Chambolle.

Just another way the Burgundy "authorities" are messin' with ya, Oswaldo!
 
The village chunk of La Combe d'Orveau are the two non-contiguous light pink plots on the far left: http://www.kobrandwineandspirits.com/education/vineyard_maps/france#39 You'll notice that the far larger chunk of the village plot isn't even contiguous to the 1er cru part. However, you'll also notice a totally different example of just what you were talking about: there is a village lieu-dit called Les Amoureuses, which is nowhere near the famous 1er cru.

You might be thinking of Grivot - Arnoux doesn't have any labeled, though I don't know whether he owns any that might go into the village Chambolle.
The Mugnier village Chambolle is about half village Combe d'Orveau and half 1er cru Les Plantes if I remember right.

Indeed, from Clive Coates: "... this premier cru climat (there is also a bit of village aoc under this name, from which the Domaine Jean Grivot of Vosne Romanee produces a very good wine) is little known but can produce some extremely good wine. It is more Echezeaux than Amoureuses however, with a satisfying core of concentration and depth. But no lack of elegance either".

No, it's Grivot. Mugnier also has village Combe d'Orveaux, but it gets mixed in his village wine with premier cru Les Plantes. Arnoux has no vines there -- his Chambolle plot is further north -- but southwest of the village.

Although there technically is a village lieu-dit called Amoureuses, I've never seen a bottle of Chambolle with that on the label, and in fact when I asked Christophe Roumier one time if there was a village lieu-dit called Amoureuses, he had to stop and think, before he responded that he "believed" there was.

It's not unusual to have a particular climat that is split into village and premier cru (in fact, Chambolle has more than a few). But Combe d'Orveaux along with Monts Luisants in Morey and Les Paulands in Aloxe are lieu-dits at grand cru, premier cru, and village level.
